Complete forms Praxis Desk

When patients have filled out a form, you review their entries and complete the form. You transfer data directly to the patient record.

Prerequisites

Required license

This feature requires the Forms add-on for Praxis and Desk.

How to do it

  1. Go to Journaling > Formulare (forms). Filters and table are described in Forms — Journaling.
  2. For a form with the status Ausgefüllt (filled out), open the action menu (icon on the right side of the row).
  3. Select Abschließen (complete) to start the review.
  4. In the detail view, compare the patient's entries with the data in the record.
  5. On the right you see the tabs Stammdaten (master data), Werte (values), Bodychart, Dokumente (documents), and Zuweisung (assignment). Offisy only shows tabs that have content.
Fig. 1The completion dialog: the entries on the left, the patient card and the assignment on the right.
  1. If the form has graded answers, the Eingestufte Antworten (graded answers) card lists them together with the question, the answer, and the grade.
  2. In the Stammdaten tab, toggle the switches next to the fields you want to transfer (e.g. address, phone number). Then click Patient aktualisieren (update patient).
  3. In the Dokumente tab, save uploaded files to the record with Datei in die Historie speichern (save file to history). Choose the Typ (type) and click Speichern (save).
  4. Click Abschließen. If required information is missing, confirm with Trotzdem abschließen (complete anyway).

Tip: In the completion window you can fill in answers directly on the form in Anmerken (Annotate) mode. For details, see Fill in directly on the form.

Note: For e-card forms with the add-on enabled, Über das e-card System übermitteln (submit via the e-card system) completes the form and opens the DBAS submission.

Tip: You can pin completed forms to the top in the patient record:

  1. In the record's forms tab, open the form's action menu. The tab name depends on your configuration, e.g. Patientenformulare (patient forms).
  2. Select Formular oben anheften (pin form to top).

The form then appears at the top of the list. Formular von oben lösen (unpin form from top) undoes it. For more on pinning, see Pinning a note entry.

Appointment and files in the form

If a form is linked to an appointment, the form view in the patient record shows a line at the top with the appointment name, date, and time, plus a calendar icon. Click it to open the appointment. Without a link, the line does not appear.

In the form's view modal, the Hochgeladene Dateien (uploaded files) card below the tabs lists all files the patient uploaded in the form, with name, size, and date. Use the open button to view a file, and in die Akte speichern (save to record) to store it in the patient record. If the patient uploaded nothing, the card says Keine Dateien hochgeladen (no files uploaded).

Note: If you edit an internally filled-out form (new version), the appointment link is kept. While editing, you can set, change, or remove the appointment in the Termin (appointment) field in the sidebar. The field only appears if a patient is assigned to the form.

If you remove the appointment, Offisy also releases the appointment of the associated patient form (the version the patient filled out).

If Offisy rejects the appointment (no longer available or missing permission), it still saves the form. The message Termin nicht zugeordnet (appointment not assigned) appears.

Common errors

  • Incomplete data: If patients missed important fields, open the action menu (icon on the right side of the row). Select An Patient senden (send to patient) to resend the form.

This discards all existing signatures — the patient has to sign the form completely again. The same action is also available in the action menu of a form filled in from the patient record: the patient then continues with the existing values.

  • Wrong patient: If a form went to a family member, open the completion view. In the Zuweisung tab, choose the correct patient under Zuweisen an (assign to) and click Zuweisen (assign).
