Change form settings Praxis Desk
Here you set the name, page flow, appearance, and font of a form. For how to use the editor, see Create and manage forms.
Where do I find the form settings?
Each form has general settings in addition to its content (fields/elements). In the editor header, click Einstellungen (settings) in the switch. The settings have three tabs.
Prerequisites
Required license
This feature requires the Forms add-on for Praxis and Desk.
Allgemein (General)
- Name des Formulares (form name) — Used in the form list and as
{FORMULARNAME}in the email subject. Required. For forms with a form type, the Name aus Formulartyp setzen (set name from form type) button is available. You can also change the name by clicking the title in the header. - Seitenfluss (page flow; Mehrseitige Formulare card, multi-page forms) — Sets how people filling out the form navigate between steps: Schritte shows one step after the other, Eine Seite shows the form continuously. New forms start with Schritte. Existing forms without a saved setting also open as a sequence of steps. You check how the page flow works in the preview: for forms with several steps, switch between Schritte and Eine Seite there (see Create and manage forms).
- Daten automatisch in die Akte übernehmen (Datenübernahme card, data transfer) — Applies to this form only: On submission, master data and values are transferred to the patient's record automatically. Empty form fields don't overwrite existing data.
Darstellung (Appearance)
- Farbe (color) — Color of the form in the patient record. Helps distinguish multiple forms.
- Textfarbe (text color) — Text color of the form in the patient record (contrast to the color).
- Seitenabstand (page margin) — Padding of the form content from the edges, in mm for top, bottom, left, and right. You can override left and right for individual steps (see Steps and sections). Top and bottom always apply to the whole form.
- Leere Felder zum Nachtragen drucken (print empty fields for manual completion; Druck card, print) — When you print or download a filled-out form, unanswered fields print as empty fields. Answered fields stay unchanged. The option doesn't apply to Thieme forms.
Schrift (Font)
- Allgemein (card) — Contains Schriftart (font) with the default "Standard (…)" and Grundgröße (base size). With the Alle (all), Desktop, Tablet, or Handy (phone) switch, you choose which level the size applies to. A dot on the switch shows a set override. Use the arrow button at the label to reset it. For more, see Set form appearance per device.
- Überschrift (heading), Fließtext (body text), Feldbeschriftung (field label), and Eingabewert (input value) (one card each) — Each card has Schriftart and Größe (size). On Überschrift, the size field is called Größe (H1).
You see earlier states of a form in the change history. For how to restore a version, see Change history and restoring versions.
Print empty fields for manual completion
You'll find the setting under Einstellungen > Darstellung > Druck. Turn on Leere Felder zum Nachtragen drucken there. The switch is off by default.
When you print or download a completed form, unanswered fields then appear as empty fields: as an input field, circles, checkboxes, or a signature field. In a matrix, the unanswered rows get circles. This lets you fill in missing information on paper. Answered fields stay unchanged. Nothing changes on screen, where Offisy still shows "-".
Fields hidden by a condition or with Druckformat (print format) set to Nicht anzeigen (hide) still don't appear in the printout.
The setting is also available in templates. When you create a form from a template, Offisy carries the setting over.
It doesn't apply in these cases:
- The setting takes effect for completed forms you create after you save it. Existing completed forms print as before.
- If you print annotations too, the form prints as before.
- Thieme consent forms print as before.
- File upload and measurement calculators (for example BMI) stay unchanged.
