Thieme consent forms Praxis Desk
Thieme consent forms are licensed patient information and consent forms from the publisher Thieme Compliance. You use them as a ready-made template.
Definition
Unlike your own form templates, Thieme forms come from an external publisher. You don't create them yourself.
Note: Offisy imports and updates Thieme forms centrally. You don't upload them yourself — new or revised forms automatically appear as a template in your form list. For creating forms from templates, see Create forms from templates.
Required license
This feature requires the Formulare erweitert + Thieme Aufklärungsbögen add-on for Praxis and Desk.
How do you recognize a Thieme form?
Offisy marks forms created from a Thieme sheet in several places:
- In the template list, the sheet carries the Thieme badge along with Bogen {Code} (sheet code) and Auflage {Auflage} (edition).
- In the form itself, the header shows the Thieme Compliance logo, the sheet badge, and the form title.
- This marking appears on screen and in the printout or PDF.
Editing is restricted
You can't freely edit a form created from a Thieme sheet in the form editor like your own template. The publisher sets content, fields, and layout. You can still add annotations, see Annotate a form.
Previewing the blank template stays blocked too. Because that leaves only the Ausfüllen (fill in) mode available, the preview editor hides the switch between preview modes entirely — the Ausdruck (print) mode doesn't even appear as an option. For the preview modes, see Check the form in the preview.
Note: A form that's already filled out and completed keeps the edition it was created with. If Thieme revises the sheet, Offisy creates a new version of the template. Already completed forms stay unchanged at their original edition, and new forms are created from the new edition from then on.
Note: If a practice's Formulare erweitert + Thieme Aufklärungsbögen add-on isn't active, Offisy skips its templates during an edition change. Once the license is active again, Offisy catches up the templates automatically — without any action from the practice.
What are Thieme consent forms used for?
You fill in Thieme forms for patients, print them blank, or review earlier editions.
Print and download a blank form
Even if you don't fill in the sheet digitally, you can print or download it as a blank form. In the form list, open the action menu of a Thieme form and choose Leerformular drucken (print blank form) or Leerformular herunterladen (download blank form).
In the modal, use the Anzahl (number of copies) field to set how many copies you need (1 to 10).
When you print, Offisy creates the PDF and your browser's print dialog opens right away.
Each sheet carries a vertical strip on the left margin with this information:
- The note "For single use only"
- Date and time
- Sheet and edition
- Receipt number
- Page number
Each copy gets its own receipt number.
Note: If the Formulare erweitert + Thieme Aufklärungsbögen add-on isn't active, clicking either menu item shows a license notice instead of opening the modal.
Print and download from the patient record
When you print or download a Thieme form from the patient record or from Journaling > Forms, it gets the same margin strip with a receipt number as the blank form. This applies to both filled-in and blank sheets, and each output gets its own receipt number.
When you print, your browser's print dialog opens right away.
View the edition history
To see which editions existed, choose Auflagenhistorie (edition history) in the action menu of a Thieme form in the form list. You see the editions, newest first, with the changed fields.
