Review and complete a Thieme form Praxis Desk
A Thieme consent form runs in two steps: the patient fills it out and signs it, then the treating practitioner reviews it and completes it.
Prerequisites
Required license
This feature requires the Formulare erweitert + Thieme Aufklärungsbögen add-on for Praxis and Desk.
- A Thieme form sent to the patient.
How to do it
- Open Journaling > Formulare and select the form with the status Filled out. Offisy sets this status once the patient submits the form.
- Go through the form together with the patient.
- Optional: Switch to Anmerken (Annotate) in the form and draw in what you discussed. For details, see Add annotations to a form. In Anmerken mode you can also fill in answers directly on the form. This works for every form, not only Thieme forms. For how it works, see Fill in directly on the form.
- Optional: Click An Patient senden so the patient finishes filling out the form. Confirm the prompt Formular an Patient senden with Senden. This discards all existing signatures — the patient has to sign the form completely again.
- Open the form again if you sent it back. Check all the answers and correct them if needed. Add a missing required signature, if the form requires one.
- Click Complete. If other answers are missing, click Trotzdem abschließen.
Use In neuem Tab öffnen (Open in new tab) next to the switch to open the form as its own page. If the window was in Anmerken mode, the tab starts in Anmerken mode with the drawing toolbar. Otherwise the tab shows the view of the form. Click Anmerken at the top right to show the drawing toolbar. If you have unsaved changes, Offisy alerts you.
You can also assign the form to a patient record using Assign.
If a required field is missing, Offisy points this out before completing the form. A missing required signature blocks completion entirely.
Step-by-step layout
A Thieme form can consist of several steps while filling it out. Offisy then shows them one after the other: the patient sees only one step at a time and navigates using the Next and Back buttons at the end of each step. In the last step, the Complete button replaces Next.
Conditional questions
Some questions in the form trigger further questions — only the answer given decides whether an additional question appears. A question hidden this way doesn't need to be answered, even if it's set up as required.
Recognizing critical and medium answers
Some answers are classified by the publisher as Critical or Medium — for example, answers about risk factors. You don't see this classification while filling out the form. Only during the review does Offisy show the practitioner all classified answers together in the Eingestufte Antworten (Classified Answers) card, each with the question and the answer given.
