What is the Forms module? Praxis Desk
The Forms module lets you run a paperless practice. You can create custom digital forms and send them directly to your patients or have them filled out on-site.
You sync the collected data directly with the patient record, which significantly reduces administrative overhead.
How it works
With the form editor, you design your own templates for anamnesis sheets, consent forms, or patient data sheets using drag-and-drop. You can:
- Send forms by email to individuals or groups.
- Provide forms via QR code or link in the practice.
- Automatically attach forms to online appointment bookings.
When you use it
- New patients: To digitally capture patient data before the first appointment.
- Consent: For legally required consent forms (e.g. GDPR, treatment agreements).
- Anamnesis: To collect medical information in a structured way.
- Marketing: For newsletter sign-ups.
