Share forms with patients Praxis Desk

You can send forms to your patients in several ways so they can fill them out on their own device (smartphone, tablet, PC).

Prerequisites

  • The form has been created in Settings and enabled for the patient record.
  • For email delivery, a valid email address must be stored in the patient's record.

How to do it

Option 1: Via the patient record (individual send)

  1. Open the desired patient record.
  2. Click the Forms button.
  3. Select the form you want.
  4. Choose one of the following options:
    • Fill out yourself: When the patient is in front of you and you/they fill it out on the practice device.
    • Send form: To deliver it by email.
    • QR code/Link: So the patient can scan the code with their phone and open the form.

Option 2: Via Journaling (bulk send)

If you want to send a form to many recipients at once (e.g. a new GDPR consent form):

  1. Go to Journaling > Forms.
  2. Click + Create in the top right.
  3. Select the form.
  4. Add recipients via the search field, enter them manually, or upload a CSV file.
  5. Click Send.

Option 3: From the waiting list

You can also send forms directly from the waiting list:

  1. Open the waiting list and click the action menu next to the desired patient.
  2. Select Create document.
  3. Choose the desired form from the list.
  4. The form is sent with the appointment context.

Note: You can set the default email text and link validity period under Settings > Forms > Settings.
