Share forms with patients Praxis Desk
You can send forms to your patients in several ways so they can fill them out on their own device (smartphone, tablet, PC).
Prerequisites
- The form has been created in Settings and enabled for the patient record.
- For email delivery, a valid email address must be stored in the patient's record.
How to do it
Option 1: Via the patient record (individual send)
- Open the desired patient record.
- Click the Forms button.
- Select the form you want.
- Choose one of the following options:
- Fill out yourself: When the patient is in front of you and you/they fill it out on the practice device.
- Send form: To deliver it by email.
- QR code/Link: So the patient can scan the code with their phone and open the form.
Option 2: Via Journaling (bulk send)
If you want to send a form to many recipients at once (e.g. a new GDPR consent form):
- Go to Journaling > Forms.
- Click + Create in the top right.
- Select the form.
- Add recipients via the search field, enter them manually, or upload a CSV file.
- Click Send.
Option 3: From the waiting list
You can also send forms directly from the waiting list:
- Open the waiting list and click the action menu next to the desired patient.
- Select Create document.
- Choose the desired form from the list.
- The form is sent with the appointment context.
Note: You can set the default email text and link validity period under Settings > Forms > Settings.
