Link forms with services Praxis Desk

You automate form delivery by linking forms to specific individual services. For example, a patient booking an initial consultation automatically receives the anamnesis form. This saves time and ensures all necessary information is available before the appointment.

How to do it

1. Go to Services > Individual services.
2. Find the service and open the action menu (icon on the right side of the row).
3. Select Link forms.
4. In the dialog, select the forms to be sent automatically (e.g. "Anamnesis form"). You can customize the link text in the email text field using the {LINK} placeholder.
5. Set the Notification type. Available options: Booking, Change, Cancellation, Reminder, Acceptance, Rejection. At least one option must be selected.
6. Choose the Frequency:
   • Once only — the form is sent only for the first matching appointment per patient.
   • Every appointment — the form is sent for every appointment with this reason.
7. In the Text template field, customize the text shown in the form email. Available placeholders:
   • {ANREDE} — Personal salutation of the patient
   • {FORMULARNAME} — Name of the linked form
   • {LINK} — Direct link to the form (as text)
   • {LINK:linktext} — Link with custom display text, e.g. {LINK:Click here}
8. Click Save.
