Manage and complete forms Praxis Desk

The form overview gives you a clear view of all sent and completed documents. You can review the data here and transfer it directly to the patient record.

How to do it

  1. Go to Journaling > Forms.
  2. Use the filters (status, patient, date, form name) to adjust the view.
  3. Once a form has the status Filled out, open the action menu (icon on the right side of the row).
  4. Select Complete to start the review.
  5. In the detail view, you can compare the data entered by the patient with the data in their record.
  6. Toggle the switches next to the fields you want to update (e.g. address, phone number).
  7. Click Update customer to finish the sync.

Common errors

  • Incomplete data: If the patient missed important fields, you can resend the form via the action menu (icon on the right side of the row) using the Send for editing option.
  • Wrong patient: If a form was sent to a family member, you can reassign it to the correct patient in the completion view.
