Forms – Journaling Praxis Desk
The forms journaling gives you an overview of all sent forms. You can filter, edit, and complete them from here.
Where do I find the forms journaling?
Go to Journaling > Forms.
Prerequisites
Active license for the Forms module.
Filters
Narrow down the view using these filters:
- Status — filter by current form status
- Patient — search for a specific person
- Form name — filter by the form used
- Time period — filter by send date
Data table
| Column | Content |

| Form name | Name of the sent form |
| Patient | Name of the recipient |
| Status | Current processing state |
| Send date | Date the form was sent |
| Last opened | Last access by the patient |
Status values
| Status | Meaning |
|---|---|
| Open | Form was sent but not yet opened |
| In progress | Patient has opened the form |
| Filled out | Patient has submitted the form |
| Completed | Form has been completed by the practice |
| Expired | Validity period has expired |
Actions per row
- View — shows the data entered by the patient.
- Resend — sends the form link again by email.
- Share — opens the link or QR code for manual delivery.
- Complete — starts the review and sync of the form data with the patient record. Only available when status is Filled out.
Create a new form
Click + Create in the top right, select the patient and the desired form, and confirm the send.
