DVP Form in the Patient Record Praxis Desk

A patient's DVP forms are accessible directly in their patient record — without going through the DVP Forms page.

Where do I find DVP forms in the patient record?

Open the Patient record → History/Documents → set filter to DVP Forms.

Requirements

• You have the DVP Read permission.
• The patient has at least one DVP form.

View a DVP form

Click on a DVP form in the list to open the detail view. You see all form details: patient, insurance carrier, services, diagnoses, and status.

Edit a DVP form

1. Click Edit next to the desired form.
2. The edit form opens.
3. Make your changes.
4. Click Save.

Note: Editing requires the DVP Edit permission.

Delete a DVP form

1. Click Delete next to the desired form.
2. Confirm the prompt.

Note: Deleting requires the DVP Delete permission. A form that has already been assigned to a billing submission cannot be deleted.
